Mariners Church has a policy that only dogs that are trained as a service dog will be
allowed on a Mariners campus. All service dogs that are brought to a Mariners campus
must be registered with Mariners at least 72 hours in advance. Emotional support dogs,
pets or other animals will not be allowed on any Mariners campus. Please complete and
submit this form, along with a photograph of the service dog, by email to
